Passport and Visa Solutions is an agency registered with the US Department of State and authorized to expedite passport in 1 to 3 days. In addition to the government fees PVS charges a service fees for hand-carrying passport application to the government offices.

Name Change

If your name has changed, and your passport was issued less than one year ago. · If your passport was issued more than one year ago you need to renew your passport    

Requirements

To change your name on your current passport you need to complete online form DS-5504.

2. Passport

Valid U.S. passport.

3. Photos

2x2. 

 

4. Proof of name Change 

A marriage license, a court order, and copy of your state I.D. or a driver license.

Make check or money order 
$190.00 payable to U.S. Dept. of State.
